Before you get started, I just wanted to say that I appreciate you taking the time to fill out this questionnaire. It's lengthy, but this gives me the best idea of how I can best serve you. Grab a cup of coffee and get ready to spill the tea on alllll the details of your special day. See what I did there? ;) 


Please note: This questionnaire is submitted before the booking process. This is not a booking confirmation or guarantee, rather a form to begin the booking process. At the time of booking, a 30% non-refundable deposit is required along with a signed contract via email. I typically respond to emails within 1-3 business days!


I love hearing different love stories. How did you meet? Who made the first move? How did you know your partner was the one?

Remember to include detail if our location will be changing throughout the day. (where you'll be getting ready, ceremony location, reception location, etc.)
